Understanding eForm Phases

On the My Dashboard tab in the Transition Documents Reported For Me table, the Status/Phase column shows where your eForm is in the transition program. Each of the four (4) phases and its activities are described in this topic.

Pre-Separation Counseling

  • Status is In Progress.
  • Phase is Pre-Separation Counseling.
  • Service Member clicks Initialize Pre-Separation Counseling and begins the process of filling in an eForm. Sometimes the counselor starts the form for you.
  • Service Member may work by themselves or with a counselor to fill in the sections of the eForm.
  • Service Member can click Save and add Remarks during this phase and continue entering information into the eForm.
  • Service Member signs the eForm.
  • Counselor reviews and signs the eForm, which starts the Capstone Review phase.

Capstone Review

  • Status is In Progress.
  • Phase is Capstone Review.
  • Service Member and counselor complete the Capstone Review phase.
  • Counselor introduces Service Member to training or a new employer or a job site or other career opportunity.
  • Service Member signs Capstone Review.
  • Counselor signs Capstone Review, which puts the eForm into the Commander's Verification phase.

Commander's Verification

  • Status is In Progress.
  • Phase is Commander's Verification.
  • Commander or Commander's Designee signs the eForm providing final approval.
  • The status and phase change to Complete when the form is signed.

Complete

  • Status is Complete.
  • Phase is Complete.
  • Completed Date shows in table.
