The OMPF Requests section on the Request Personnel File tab displays your processing and completed OMPF requests.
For a completed Navy and Marine Corps requests, you can send a follow-up message directly to the Service Repository regarding document issues. For example, if you did not receive all of the files you requested, or one (1) of the documents has incorrect information, you can send a follow-up message to correct these problems.
Send FUM to Air Force: You can fill out a form SF180 to request the documents that you did not receive. The SF180 can be faxed to (210) 565-3124 or emailed to DP2SSM.Milrecs.Incoming@us.af.mil. For further information users can call the Total Force Service Center at (800) 525-0102 or visit the Request Military Personnel Records Using Standard Form 180 page of the National Archives website.
Send FUM to Coast Guard: If you did not receive all of the files requested, please contact HQS-SMB-CGPSC-MR-CustomerService@uscg.mil for further information. Please include your request ID, name, and submission date. Do NOT include SSN or other sensitive information.
