Starting a New Pre-Separation Counseling eForm
You can start a new eForm only if you do not already have an in-progress eForm. You can work on only one eForm at a time. If you already have a Capstone Review or Commander’s Verification eForm and you cannot edit the outdated information on it, contact a counselor to unlock it back to the Pre-Separation Counseling phase.
To start a new form:
- On the My Dashboard tab, click Initialize Pre-Separation Counseling. The system displays the Privacy Act Statement.
- Click OK. The system displays the eForm on the Transition Documents tab.
- Some fields are filled in automatically with information from your records with the DoD.
- The form contains multiple sections, indicated by a section number and name.
- Clicking a section's name expands it to show its fields.
- To save the form at any time, click Save at the top or bottom right corner.
- You must answer all required editable questions before you can sign the form.
- Certain fields are disabled and only editable by a Transition Assistance Counselor.
What you can do
- You can edit, save, and print the in-progress eForm.
- You can close the eForm and then reopen it to add more information.
Consult your counselor if you need help filling in a field or answering a question.
About the eForm
- After you begin a new eForm the My Dashboard tab shows the eForm link in the Document column of the Transition Documents Reported For Me table.
- The eForm is In Progress and the Status/Phase is Pre-Separation Counseling.
- You can click eForm on the My Dashboard tab at any time to open your eForm.
- You can sign the eForm when you have entered information in all required editable fields.
