Starting a New Pre-Separation Counseling eForm
You can start a new eForm only if you have no existing eForms in progress. You can work on only one eForm at a time.
To start a new form:
- On the My Dashboard tab, click Initialize Pre-Separation Counseling. The system displays the Privacy Act Statement.
- Click OK. The system displays the eForm on the Transition Documents tab.
- Some fields are filled in automatically with information from your records with the DoD.
- The form contains three (3) sections, indicated by a section number and name on the collapsible blue bar.
- Click a section name to show or hide the fields in the section.
- To save the form at any time, click Save at the top right.
- If you save before completing all three (3) sections, a message displays indicating that the form is incomplete. You must save the form without any errors to sign it.
What you can do
- You can edit, save, and print the eForm in progress.
- You can close the eForm and then re-open it to add more information.
Consult your counselor if you need help filling in a field or answering a question.
About the eForm
- After you begin a new eForm the My Dashboard tab shows the eForm link in the Document column.
- The Status is In Progress and the Current Phase is Pre-Separation Counseling.
- You can click eForm on the My Dashboard tab at any time to edit your eForm.
- When you have entered information in all required fields, you can sign and lock the eForm.
