Disenrollment or Loss of Coverage Letters

TRICARE Disenrollment Letters are available anytime
a voluntary or involuntary medical or dental program disenrollment occurs.
Letters are available to sponsors and their affected
family members who are in the following populations:
- Active duty, National Guard or Reserve Service
members on Active Duty or Early Alert disenrolled from TRICARE Prime
or Prime Remote, TRICARE Overseas Prime or Overseas Prime Remote,
and/or the Remote Active Duty Dental Program
- Selected Reserve Service members disenrolled
from the TRICARE Reserve Select (TRS) Program
- Retired Reserve members awaiting retired pay
at age 60 and disenrolled from the TRICARE Retired Reserve (TRR) Program
- Retired and Medal of Honor Service members disenrolled
from TRICARE Prime
- National Guard or Reserve Service members disenrolled
from TRICARE Prime during a Transitional Health Care period (TAMP)
- Dependents disenrolled from TRICARE Young Adult
(TYA), including TYA Standard, TYA Prime, TYA TRS, and TYA TRR
- Beneficiaries losing coverage for TRICARE Standard,
TRICARE For Life (TFL), TRICARE Plus, and
TRICARE Prime with a US Family Health Plan (USFHP) Primary Care Provider.
This applies ONLY when the beneficiary has NO other TRICARE or Direct
Care eligibility, in other words, a complete loss of eligibility for
DoD health care such as the result of sponsor separation or the remarriage
of a widow previously covered by TFL.
Sponsors and Surviving
Spouses can view, print, and save disenrollment letters for
themselves as well as for their family members (children) under the age
of 18.
Family Members
over the age of 18 can sign in to milConnect to access their disenrollment
letters if they have a DoD Self-Service Logon (DS Logon).
