Changing Your Primary Care Manager (PCM)

Depending on your status, address, and plan choice, you might be able to change the PCM assigned to you and your family members.

Depending on the plan type, you might not see every page described below. If so, just skip to the next step showing the page you are viewing. For TRICARE Prime Remote Plan overseas, the PCM will be assigned by your regional contractor.

  1. In the Family Members list, select the family member whose PCM you want to change.

Some plans do not allow you to change your PCM online. In these cases, the system displays a message telling you what to do next.

  1. On the Medical Enrollments tab, click Change PCM.
  2. Read and select all of the checkboxes.

Note:  Select the access standards link to learn about TRICARE Prime distance and drive time standards. You will need to waive access standards to enroll online. You can also select the paper form link to download a paper form and mail it to your contractor.

  1. Click Continue.
  2. On the Change PCM page, select the reason that you want to change your PCM and select the date to end care with your current PCM.
  3. Click Continue.
  4. On the Change PCM - Select Provider Type page, select a provider type:

If you select Military hospital or clinic:

If you select Civilian Health Care:

If you select USFHP Facilities:

  1. On the Change PCM - Search for a PCM page, enter search criteria and click Search.

If no PCMs are found, enter different search criteria. Otherwise, the system displays a list of PCMs matching your search criteria.

  1. On the Change PCM - Assign a PCM page, select the PCM that you want.  Click Continue.
  2. On the Change PCM - Confirm PCM Changes page, confirm the details.  Click Submit.

A message tells you that your request to change your PCM was successful. Read the information to find out:
