Generating Eligibility Letters
Select Correspondence/Documentation, eCorrespondence, Proof of Coverage tab
- On the Proof of Coverage page, do one of the following:
- If you are a sponsor, select the name(s) to be included on the letter by clicking the checkbox next to each name. Only five (5) names (sponsor and family members or family members only) can be included on a single Eligibility Letter. You can generate additional letters as needed.
- If you are a family member, select the checkbox next to your name.
- Click Generate PDF. A File Download dialog opens.
- In the File Download dialog:
- Click Open to display the letter in a new Adobe Reader window. You can use the Adobe Reader functions to print and/or save the PDF file.
- Click Save to save the PDF file to your computer.
Note: Adobe Reader is required to open an Eligibility Letter PDF file. You can download the free Adobe Reader from the Adobe website. Follow the instructions to download and install Adobe Reader.
